Assistance is provided to low-income individuals and people with special health care expenses because everyone deserves access to quality care, regardless of your financial situation. We will not deny service based on your ability to pay. No one will be turned away in a crisis or emergency situation.

Anyone can apply for financial assistance. Please contact us at 541-426-4524 and we will get you started on our sliding fee scale based on your income.
Who qualifies for financial assistance?
It depends. Sliding Fee is based on your total household income and the number of people in your household. The Federal Poverty Guidelines are used in creating the sliding fee schedule to determine eligibility (this is updated annually). You are required to provide proof to substantiate the income you report in the application. Proof of income can be either 3 months worth of paystubs, most recent tax returns, copy of Social Security or Disability letter, copy of unemployment benefits letter or a combination of these depending on the client's source of income.
Who covers the sliding fee?
The state of Oregon pays the difference between what you are charged and the full rate of service.
